COVID PASS MEDICAL EXEMPTIONS - there are only very limited medical reasons why a patient cannot have the covid vaccination and only these patients will be eligible for a medical exemption pass.  You MUST contact 119 to ask for an application form.  If you are eligible, the 119 service will send you a form.
